مقدمه:

نوعی بیماری شایع است و با نسبت 1در هر 3000تولد بروز میکند.

• حداقل 8 فرم نوروفیبروماتوزیس شناخته شده ست

• شایع ترین نوع آن ، تیپ 1 می باشد.

• نوع توارث اتوزومال غالب

• علت اختلال: موتاسیون در ژن NF1در کروموزوم 17 بیماران نوروفیبروماهای متعدد دارند که در هرناحیه از بدن میتوانند ایجاد شوند ولی بیشتر پوست را درگیر میکنند.

• نمای بالینی: از پاپول های کوچک تا ندول های نرم بزرگتر تا توده های قابل حرکت و متورم وسیع روی پوست (نوروماتوزای فیلی) متغیر است.

• نوع پلکسی فرم نوروفیبروما که در لمس شبیه یک کیسه پر از کرم است پاتوگنومیک ضایعه می باشد.

درصورتیکه بیمار دو یا چند ویژگی زیر را داشته باشد معیار تشخیص نوروفیبروماتوزیس محقق شده است:

تعداد 6 یا بیشتر ماکول شیرقهوه ای با قطر حداقل بیش از mm5 در افراد نابالغ و قطرحداقل بیش از 15 mmدر بالغین. دو یا چند نوروفیبروما از هر نوعی یا یک نوروفیبرومای پلکسی فرم.

وجود لک در ناحیه زیر بغل و کشاله ران گلیومای اپتیک دو یا چند ندول لیش (هامارتوم عنبیه) .ضایعه مشخص استخوانی مثل دیسپلازی اسفنوئید یا نازک شدن کورتکس استخوان دراز همراه یا بدون سودو آرتریت. یک خویشاوند درجه یک مبتلابه نوروفیبروماتوزیس تیپ1 بنابر معیارهای بالا.

• شایعترین یافته دهانی، بزرگ شدن پاپیلاهای قارچی است.(تقریبا در 50درصد بیماران)

• یافته های رادیو گرافی شامل بزرگ بودن سوراخ مندیبولار،بزرگ بودن یا شاخه شاخه شدن کانال مندیبولار،افزایش دانسیته استخوان،تقعر سطح داخلی راموس و افزایش ابعاد کورونویید ناچ می باشد.

• گاهی اوقات این ضایعه میتواند باعث تورم یکطرفه صورت مشابه هیپرپلازی نیمه صورت شود.

The aim of this book is to introduce and develop an arithmetic analogue of classical differential geometry. In this new geometry the ring of integers plays the role of a ring of functions on an infinite dimensional manifold. The role of coordinate functions on this manifold is played by the prime numbers. The role of partial derivatives of functions with respect to the coordinates is played by the Fermat quotients of integers with respect to the primes. The role of metrics is played by symmetric matrices with integer coefficients. The role of connections (respectively curvature) attached to metrics is played by certain adelic (respectively global) objects attached to the corresponding matrices.

One of the main conclusions of the theory is that the spectrum of the integers is “intrinsically curved”; the study of this curvature is then the main task of the theory. The book follows, and builds upon, a series of recent research papers. A significant part of the material has never been published before.
